Salary: £17,550 - £19,550 per annum (yearly increments) 
Contract Hours: 36 hours per week

Role Overview

To take a lead role in supporting key workers to complete assessments and person centred planning tools.

To take part in preparing support plans and ensure support packages are established, delivered, monitored and reviewed at appropriate intervals.

To ensure all relevant regulations, legislations and organisational requirements are met whilst on duty and deputise in the absence of the registered manager.

To carry out support / action plans to protect service users at risk, carrying out any preventative activities in line with the Safeguarding of Vulnerable Adults policies and procedures.

To monitor and feedback on the standards and quality of care provided to service users through effective communication and quality review systems, conforming to Premier Care (Plymouth) Limited's policies and procedures.

To provide direct support services where appropriate

Essential Criteria
  • Driver with access to own vehicle
  • Able to cover shifts at short notice & to take on-call duties (rota'd within management team)
  • Experience within Health & Social Care 
  • Experience with managing staff
  • Confident / able to work well under pressure

The role is subject to a satisfactory DBS check and at least two satisfactory references, one of which must be from your most recent employer.
